NHS Social Prescriber

Social prescribing is the non-clinical and non-urgent support within your GP practice. Social prescribers give you time to focus on what matters to you by taking a holistic approach to your health and wellbeing. The social prescriber is in the cafe twice a month and they can help with any of the following issues: 

- Benefits / finance form filling 
- Bereavement support 
- Carers services 
- Social activities / community groups 
- Counselling options 
- Domestic abuse 
- Social isolation / loneliness 
- Emotional wellbeing support 
- Advocacy support 
- Housing support 
- Lifestyle (for example weight management, smoking, alcohol / substance abuse) 
- Volunteering / training opportunities
